Colorectal cancer

Primary site, DNA repair and treatment history guide the interpretation of colorectal research.

Editorial update · October 6, 2026

Context and biological diversity

Colon and rectal cancers share mechanisms but differ in location, management and relevant outcomes. Data organization should distinguish these sites, stage, histology and hereditary context. Microsatellite instability and repair deficiency are tumor characteristics that require explicit definitions and testing methods.

What research makes it possible to study

TCGA described hypermutated and non-hypermutated tumors and recurrent alterations in pathways such as WNT and RAS. The observed molecular convergence does not erase anatomical or exposure differences. These findings help formulate stratified analyses and examine links between repair mechanisms and patterns of genomic alteration.

Application in a research project

A biomarker study can prespecify repair status, location and treatment, avoiding the combination of groups with different prognoses. A prognostic marker must be distinguished from a marker that modifies an intervention’s effect. Screening studies should assess incidence, mortality, false positives and subsequent procedures in people without a diagnosis.

Research questions

Does a biomarker’s association with recurrence persist after accounting for site, stage and repair status?

Study design
A cohort with prespecified variables and analysis; validation in a separate dataset without redefining the cutoff.
What to measure
Recurrence, follow-up, the proportion of inconclusive tests and performance in anatomical subgroups.
Interpretation limits
Resection material may select operable cases. Evidence from colon cancer should not be transferred wholesale to rectal cancer or hereditary syndromes.
