Context and biological diversity
Ductal adenocarcinoma and pancreatic neuroendocrine tumors are different entities. The molecular evidence highlighted here concerns ductal adenocarcinoma. Stage, resectability, sample type and the proportion of tumor cells need to accompany comparisons between patient series.
What research makes it possible to study
TCGA characterization showed recurrent alterations in KRAS, TP53, CDKN2A and SMAD4 and highlighted the challenge of low neoplastic cellularity. Tumor material also contains stroma and immune cells; a molecular profile may reflect this mixture. This observation informs quality control and interpretation of expression signatures.
Application in a research project
To investigate early detection, comparing advanced cases with healthy people is insufficient. It is more informative to define the intended-use population, collect material before diagnosis and include benign conditions that can produce positive results. The protocol should explain case confirmation and follow-up of participants with negative results.
Research questions
Does a biomarker panel distinguish early disease from benign pancreatic conditions in a defined at-risk population?
- Study design
- A prospective accuracy study with an independent reference test, blinded assessment and follow-up of negative results.
- What to measure
- Sensitivity by stage, specificity, positive predictive value and additional procedures triggered.
- Interpretation limits
- Low prevalence reduces positive predictive value. Retrospective performance in selected samples does not establish screening benefit.
Interpretation limits
