Soft tissue sarcomas

Numerous histologies and distinct molecular mechanisms make a single conclusion for all sarcomas inappropriate.

Editorial update · October 6, 2026

Context and biological diversity

Soft tissue sarcomas arise in different tissues and include entities with their own molecular alterations and behavior. Location, grade, histology and extent need to be described. Diagnostic review and the classification version used are especially relevant when data from multiple centers are combined.

What research makes it possible to study

TCGA studied a defined set of adult sarcomas and found genomic and epigenomic diversity, with copy-number alterations playing an important role in several groups. The cohort does not cover all histologies. Its findings offer subtype-specific hypotheses and do not support the idea that any identified target has proven clinical efficacy.

Application in a research project

A multicenter registry can harmonize names, reports and outcomes while keeping each subtype identifiable. Basket studies require a rationale for sharing evidence across tumors and an analysis of heterogeneity. In small groups, uncertainty intervals and participant descriptions are essential to avoid treating a few events as stable conclusions.

Research questions

Can data be combined across centers without losing the definition of histological subtypes?

Study design
A prospective registry with a common dictionary, eligibility review and histology-stratified analysis.
What to measure
Diagnostic agreement, completeness, participant numbers per subtype and outcomes at a defined time.
Interpretation limits
Excessive grouping can hide divergent effects. Evidence from selected adult sarcomas does not represent all pediatric mesenchymal neoplasms.
